OBTAINING CERTIFIED COPIES OF VITAL RECORDS

NH Vital records are considered to be private and access to them is restricted to those individuals with a “direct and tangible interest”.  The applicant, a member of his/her immediate family, his/her guardian or respective legal representatives shall be considered to have a “direct and tangible interest” when information is needed for determination or protection of a personal property right.

“Immediate Family” means:  a person related either by blood or marriage and includes the following:  mother, father, son, daughter, brother, sister, husband, wife, grandfather, grandmother, grandson, granddaughter, great-grandchild, step-parents, step-children, aunts, uncles, nephews and nieces (no cousins).

The Town Clerk’s Office is not limited to issuing only certified copies of birth, death, marriage and divorce records for events that occurred in Madison/Silver Lake, New Hampshire.  Through the Division of Vital Records, we have access to statewide records for births beginning with 1984; deaths beginning with 1990 and marriages beginning with 1989.  Statewide civil union records may also be requested through this office as well as statewide divorce decrees from 1990 to within six (6) months from the present search date.  For divorce records prior to 1990, please contact the courthouse where your divorce took place. 

FEE:  A $15.00 fee applies for the issuance of one certified copy of a vital record; additional copies purchased at the same time are $10.00 each.  Please be aware that New Hampshire law requires a nonrefundable search fee of $15.00 be collected for each record requested.  If the record is located and you meet eligibility requirements, you will be issued the requested number of certified copies of that record.   Cash, Checks, Credit/Debit cards are accepted for payment.

Please note that a valid photo ID is required in order to process your request; a legible photocopy of the applicants government issued photo ID should be submitted along with your request.

 The Town Clerk’s Office is able to search for specific records only.  The state fee for a vital record search, including a copy of the record is $15.00.  Additional copies of the same records can be purchased at the same time for $10.00 each.
